What
are Dental Implants?
Replicating the natural function and appearance of your lost teeth
can be very difficult to accomplish. Historically, dentures or bridge
restorations would be used as replacements, but with limited results.
Dental implants, however, are natural-looking replacements for missing
teeth that also provide the same function as your natural tooth
root. They have also been used to anchor these other types of restorations
for greater success and patient satisfaction.
A More Natural Approach
A dental implant is a small, sturdy, titanium post that acts as
the root structure would for a natural tooth. A dental implant is
placed into your upper or lower jaw bone. After the bone has grown
around the implant, implants can hold a crown, bridge or over-denture
just like roots hold natural teeth in place. Implants are very durable
and can last a lifetime. They require the same maintenance as natural
teeth; this includes brushing, flossing and regular dental check-ups.
A single tooth or a full arch of teeth which have been lost due
to injury or disease can be replaced with dental implants. Titanium
metal is used because of its compatibility with bone and oral tissues.

Dental
Implant History
Dental
Implants were developed in 1952, in a laboratory in the university
town of Lund, Sweden by Professor Per-Ingvar Brånemark, who
had a “lucky” accident during bone grafting research.
Much to his dismay, Dr. Branemark discovered that
it was impossible to recover any of the bone-anchored titanium microscopes
he was using. The titanium plate had apparently bonded irreversibly
to living bone tissue, an observation which contradicted all scientific
theory at the time.
Dr. Branemark went on to demonstrate that under
carefully controlled conditions, titanium could be structurally
integrated into living bone with a very high degree of predictability,
and without long-term soft tissue inflammation or rejection. Brånemark
coined the name “osseointegration,” meaning the attachment
of healthy bone to a titanium implant.
Thus dental implants were born and the first application of dental
osseointegration was the implantation of new titanium roots in an
edentulous (toothless) patient in 1965. Dental implants have shown
a 90 percent success rate and long-term studies continue to show
improving success.

What
are the types of Implant Restoration
There are three main categories that need to be distinguished:
- Fixed Prostheses
- Removable Prostheses
- Fixed-Removable Prostheses
1. Fixed Prostheses are, as the name implies,
fixed to the dental implant and the patient cannot remove them (although
the doctor usually can). Within this category fall single-tooth
as well as multiple-tooth replacements. Fixed Prostheses are usually
the most desirable prostheses in implant reconstruction, however
depending on the individual situation that may not always be indicated.
Multiple teeth in a row are usually splinted together in a Bridge,
which is then attached to the implants. Below you see several examples
of Fixed Prostheses.

2 Removable Prostheses are attached
to implants in a way that permits the patient to remove them at
any time. This usually also implies that the prosthesis is not completely
supported by dental implants, but rather by the implants and the
tissues in your mouth. In fact they are usually your typical Full
Denture or Partial Denture that derive some extra retention out
of the implants. As mentioned above, one usually tries to strive
for a Fixed Restoration, however in certain situations compromises
have to be made and this type of restoration can still benefit the
patient tremendously. Below you can see again several examples of
Removable Prostheses for implants.

3. Fixed-Removable Prostheses are usually found
in full mouth reconstruction for patients who have either one or
both jaws without teeth. This prosthesis is usually completely supported
by implants (with no or little tissue contact-just like a Fixed
Prosthesis), however the patient or the doctor can remove part of
the prosthesis for hygiene access. This entire set-up usually consists
of two parts: a retentive metal part (usually some type of bar attached
to the implants) which is not removable and the actual super structure
which has the teeth on it, which is removable. This super structure
fits over the metal part (connected to the implants) and it usually
stays connected to that metal part via friction mechanisms and/or
retentive clips.

implant), they actually become part of your body!!!!

Who
Is A Good Candidate For Dental Implants?
- Anyone who is missing one or more of their teeth.
- Age Is Not A Factor.
- Osteoporosis Is Not A Factor - provided there is sufficient
bone in which to anchor an implant. Bone grafts are a possible
option.
- In general, anyone who is healthy enough to undergo a routine
tooth extraction or oral surgery is probably a good candidate
for dental implants.
- Individuals with certain chronic diseases (e.g., diabetes),
heavy smokers, and alcohol abusers may not be good candidates
for dental implants.

How
Successful Are Implants?
- Factors such as patient health, implant location, amount and
quality of bone, and patient home care all affect dental implant
success.
- Under favorable conditions, dental implant success rates vary
between 90% and 98%, according to most published studies.

What
Does The Procedure Involve?
Phase I - Preparation
Study models of the jaw are made from molds, or impressions, of
the mouth. A surgical guide is then made from the mold. Special
x-rays of the jaw are also taken - both an individual x-ray (periapical)
and an x-ray of the entire mouth (panoramic).
If there is not enough bone to place the implant, a bone graft
may be required. Grafts are usually taken from the hip, a different
area of the jaw, or the skull. Donor bone may also be used.
Phase II - Placing The Implants
Dental implants are then placed. Local anesthesia is sufficient
for most patients. The implant area is anesthetized, and an incision
is made in the gum to expose the jaw. The surgical guide made from
the jaw mold is then placed over the jaw to aid in the placement
of the implant.
A series of successively wider drill bits are used to create an
opening in the bone.
The implant is placed in the jaw by screwing it into the opening
made by the drill so that the top of the implant is at or slightly
above where the jawbone meets the gum line. A cover (healing screw)
is then placed in the implant to prevent debris from filling it.
The gum is then stitched over the healing screw, and the Phase
II is over.
After implant placement, dentures cannot be worn for about 2 -
5 days. Antiseptic mouthwash, antibiotics, and pain medication are
all provided to prevent infection, and for the patient’s comfort.
Phase III - Osseointegration: Fusing To The Bone
Time is required to allow the bone to fuse with the implant- a
process called osseointegration. Depending on the where the implants
are placed, this process can take anywhere from 3-6 months for the
lower jaw and 6-9 months for the upper jaw.
Phase IV - Cosmetic Prostheses: Finishing Up
All procedures from this point on are much easier than for natural
teeth, and require no anesthesia.
Depending on the individual case, a variety of devices can be
attached to the implant. This process usually requires one visit
to take impressions for fabrication, and a final visit to place
and adjust the teeth or tooth.

Does
Dental Implant Surgery Hurt?
Because each surgery is different, and each patient has a different
pain threshold, this is a difficult question to answer.
Most patients report less pain with a dental implant than a simple
tooth extraction. Many report no pain at all.

How
Long Do Dental Implants Last?
Although dental implants are designed to last a lifetime, the
earliest implant is only about 30 years old. The overall dental
health of the patient, the condition of the bone structure of the
jaw, and regular follow-up care by your dentist all play an important
part in how long implants last.
According to published studies, the survival rate after 5 years
is 95% for implants in the front of the lower jaw and about 85%
for implants in the sides and rear of the upper jaw. Few, if any,
implants fail completely.
